We are excited to inform you that we have created a new patient portal to improve your patient experience at The Stratford Clinic, designed to make managing your healthcare more straightforward and supportive.

ACTION YOU NEED TO TAKE: You MUST set up your portal account prior to your next appointment. If you do not create a portal account, your appointment may need to be rescheduled.

1. Create Your Patient Portal:

1. You should have received a Client Portal Invitation email from “TherapyPortal.com”; please check your junk folder on your email if you are not seeing the invitation. If you do not see an Client Portal Invitation email, please request a new invitation link this by emailing welcome@stratfordclinic.com

2. Click the link on the email “create client portal account for [your email address]” and follow the steps shown on the portal.

2. Update Your Details: Please ensure your contact and billing information is current. This includes your phone number, email address, physical address, and the party responsible for billing.

3. Complete Necessary Paperwork: Review, sign, and complete any required documents on the portal.

4. Secure Payment Information: Enter your credit card information into our secure system.

Complete Control Over Your Appointments: Directly manage your appointments through the patient portal. Please set up your portal access to utilize these new features fully.

Immediate Access to Documents: Your important healthcare documents and schedules are now instantly accessible, saving you both time and hassle. Review and sign documents.

Important Changes:

Communication: All communications, including inquiries about billing, scheduling, and medication refills, should be directed through your patient portal. This change will enhance security and efficiency. Note: Messages are not monitored outside of business hours.

Text and Email Updates: Beginning on June 1st, 2024 – All providers will communicate exclusively through the portal. Learn more about secure messaging through the portal.

After June 1st, 2024, providers will no longer use email or text message for patient communications and these methods of communications will no longer be monitored.

Additional Information for Parents and Guardians:

• If you are the parent or guardian of a minor patient or a patient who began treatment as a minor but is now 18 or older, please update your paperwork accordingly.
